CWA/Verizon Regional Work and Family Benefit Program
2026 Forms are now available for:
• Dependent Care Application and 2026 DCRF Reimbursement Form (see below for link to form)
• Must be emailed by the 2nd Friday of the following month
• $100 per family per week
• Must reapply with 2025 taxes prior to your May 2025 reimbursement request
• Pendant Program Application and 2026 Pendant Reimbursement Form (see below for link to form)
• $60 a month paid quarterly (maximum 2 per member)
Remaining 2025 Payout Dates:
• Health and Wellness - - Tentative pay out 4/24/26 - Program on hold for 2026
• College Books - - Tentative payout 5/29/26 - Program on hold for 2026
Appeal Process:
Appeals must be received within 45 days of your written notification of denial of enrollment or within 45 days of a der of reimbursement for expenses.
Appeals must be submitted to: [email protected]
• Please put: Program Name - Appeal in the subject line of the email
• You must attach all necessary documentation when filing an appeal.
• Include a valid reach number and current email address for a response.

• Tutoring
